I hereby release Miami Lighthouse for the Blind from any/all liability of whatever kind and nature which, at any time, could result from obtaining and having an employment decision based on such information.

I understand that misrepresentations, falsifications or omissions of facts requested in this application may remove me from further consideration for employment or withdrawal of any offer or employment. I understand that, if employed, misrepresentations, falsifications of any kind or omissions of facts called for on this application shall be considered sufficient basis for termination of employment at any time.

I understand that this employment application or the granting of an interview does not represent a contract of employment or a promise of future employment. I understand that should an employment offer be extended to me and I accept it, that I will fully adhere to the policies, rules and regulations of employment of the Miami Lighthouse for the Blind. However, I further understand that neither the policies, rules, regulations of employment nor anything said during the interview process shall be deemed to constitute the terms of an implied employment contract. I understand that employment at Miami Lighthouse for the Blind is at-will and it may be terminated by either party at any time, for any reason, with or without cause or advance notice.

I understand that an employment offer will be contingent upon satisfactory outcome (as determined by Miami Lighthouse for the Blind in its sole discretion) of pre-employment screening processes which may include, but are not limited to the following: criminal background investigation, drug test, driving record, verification of education, verification of employment and reference check.

I understand that this application will remain active for one year as of the date of the application. If I wish to be considered for employment beyond one year I should complete a new application for employment.
